'Convert a google form into html with file upload

I am trying to open a Google forms in HTML so i can customize its look, so far all the fields' types are OK & the submit action is working smoothly, but i have this issue: 1- the upload field action is working but seems not working as the file is not saved, once I clock on submit , it says : "some questions still need some attention" even though I made a form with only this question (file upload)

Any help or idea? Thank you in advance
