'Combine 2 table to develop piechart

I need to develop pie chart using php for Appointment Table at column Indication but indication row show int number like 2,3, / 1,2,. Therefore, indication column related with indication table which is indication id 1 for Medical, 2 for dental, 3 for Appointment and 4 for Checkup.

Does anybody have suggestions or examples of coding in php?
